Wishmaster 4: The Prophecy Fulfilled (2002)

When you come across a large lump of coloured glass, it obviously pays to be careful what you do with it, in case an ancient spirit is unleashed, which will do you a lot of harm.

At the end of the film the Wishmaster momentarily turns into Lisa; no morphing, though. I suppose the budget wouldn't stretch to that. The camera cuts away at the moment of the transition.
