The Roman Spring of Mrs Stone (1963)

Paolo wearing the hat
A rather dated film about a "society" actress. Her loving husband suddenly dies, and although at first she is devastated, she realises that she can get sexual gratification by attracting young men with her money. She stays in Rome to do so. The film follows a novel, and it shows.

After the halfway mark, one of the young men, Paolo (Warren Beatty), has been helping her shopping; we see him carrying the parcels to her bedroom, and as his hands are full, he is wearing a hat she must have bought. The scene is brief, and is filmed through a gauze curtain so it is very indistinct; and not really transgender in my opinion.
